Luxe nurses are highly trained professionals comprising of Registered Nurses (RNs) and Licensed Practical Nurses (LPNs). They coordinate and direct the patient’s care based on individual needs and deficits in the home setting. Our nurses evaluate the patient for home health eligibility and develop an individualized treatment care plan in partnership with the patient’s physicians. Our nurses will also educate patients, families, caregivers, and community providers to safely perform care that is best suited for each individual patient.

Beyond initial evaluation, treatment and care planning, our nurses provide follow-up care by continually evaluating the effectiveness of the home health care plan and modify as needed. They will monitor the patient’s needs routinely and see if the plan is effective by working with family members, caregivers, and the interdisciplinary team. Our nurses are also responsible for ensuring that appropriate referrals to other services are made, interdisciplinary conferencing is taking place regularly, and that appropriate care is provided for the patient to thrive at home.
