Our nurses are highly trained professionals compromising of Registered Nurses (RNs) and Licensed Practical Nurses (LPNs). They coordinate and direct the patient’s care based on individual needs in the home setting. Home Health Nurses evaluate the patient for home health eligibility and develop the care plan in partnership with the Physician. Our nurses will also educate patients, families, caregivers, and community providers to safely perform care that is best for each individual patient.

Beyond initial evaluation and care planning, our Nurses provide follow-up by continually evaluating the effectiveness of the home health care plan. They will monitor the patient by working with caregivers and the patient on how the plan is working. Our Nurses are also responsible for ensuring that appropriate referrals to other services are made, interdisciplinary conferencing is taking place regularly, and that appropriate documentation is completed.
